ACKNOWLEDGEMENT OF SERVICES, HIPAA & PRIVACY PRACTICES

Care Guide Inc. d/b/a ElderVizor (“CGI”)

This Acknowledgement of Services and Privacy Practices (“Acknowledgement”) governs your use of the Care Guide Inc. (“CGI”) website, intellectual property, and advisory services (“Services”). This Acknowledgement is not a contract for placement, but rather a disclosure of how CGI operates and how your information is used.

OUR ROLE & YOUR RIGHT TO CHOOSE

CGI provides complimentary senior housing and care navigation services to families. You are never charged for our services.

You always retain the right to choose any provider you wish. CGI does not sell or distribute your personal information and does not release your contact information to any provider without your explicit permission.

CGI serves as a professional filter and advocate — preventing families from being overwhelmed by unsolicited calls and ensuring recommendations are appropriate, available, and aligned with your needs.

If you contact a community independently, please inform them that you are working with CGI, as your Client Profile may already have been submitted. This ensures continuity of advocacy and preserves negotiated referral arrangements within our network.

HOW REFERRALS WORK

By choosing to work with CGI, you authorize CGI to share only pertinent, non-contact information with a limited number of appropriate providers for matching purposes. This information may include care needs, preferences, geographic parameters, and financial ranges.

CGI sends referrals only to providers that have confirmed they can potentially meet your loved one’s needs — so families are not overwhelmed with inappropriate or unavailable options.

If you later identify a provider not included in your initial recommendations, please notify your CGI Advisor so we may follow up on your behalf.

When CGI is the initial referring source, providers are contractually obligated to pay CGI a referral fee if placement occurs — regardless of how you ultimately learned of the provider. This arrangement allows CGI services to remain free to families.

SERVICES PROVIDED

CGI provides non-medical advisory services, including assistance with:

  • Independent Living

  • Assisted Living

  • Memory Care

  • Residential Care Homes

  • In-Home Care Services

Recommendations are based on:

  • Your Client Profile

  • State survey data

  • Professional interviews with communities

  • Prior client experiences

  • Information received from your care providers (with your permission)

CGI does not participate in billing, payments, contracts, or medical decision-making between families and providers.

Although CGI is compensated by providers, we act as your advocate — representing your interests in identifying appropriate care options.

CGI cannot guarantee placement, availability, pricing, or satisfaction outcomes, but we will actively assist in advocacy, coordination, and issue resolution when concerns arise.

CGI does not provide medical advice. All medical decisions must be made by licensed healthcare providers.

HIPAA & PRIVACY PRACTICES

CGI is committed to protecting your privacy and complies with the Health Insurance Portability and Accountability Act (HIPAA) where applicable.

Use of Protected Health Information (PHI)

With your consent, CGI may collect, use, and disclose limited health-related information necessary to:

  • Identify appropriate care providers

  • Facilitate referrals

  • Coordinate care placement

  • Communicate with authorized providers

Safeguards

CGI maintains administrative, technical, and physical safeguards to protect your personal and health information from unauthorized access, use, or disclosure.

Disclosure Limitations

CGI will never sell your information and will never release contact details or medical information to providers without your authorization. Information is shared strictly on a need-to-know basis for referral and placement coordination.

Your Rights

You have the right to:

  • Request access to your information

  • Request corrections

  • Limit disclosures

  • Revoke authorization in writing

  • Receive an accounting of disclosures

Retention

Information is retained only as long as necessary to fulfill service and legal requirements.
